Job Description Summary:Under the direction of the Clinical Manager, the Psychiatric Registered Nurse will provide therapeutic interventions to patients with identified psychiatric diagnoses. These interventions will be carried out following standards of practice guidelines, as well as state, federal, and accreditation guidelines. Performs crisis intervention as necessary to protect patients and others from harm on behalf of the agency and in support of visiting staff.Essential Job Functions/Responsibilities:A.Performs a complete assessment of all patients as assigned, including past and present history; physical assessment of the patient, mental status examination and evaluation of the home environment and family support systems. Accurately completes Oasis documentation as required in a timely manner.B.Develops and implements a patient plan of care in consultation with the patient’s psychiatrist and/or medical doctor.C.Coordinates total patient care with other disciplines through Clinical Manager of assigned office.D.Develops, implements, and revises nursing care plans on on-going basis as well as maintains patient’s medical record to include 485s, Oasis, case conferences, and notes daily.E.Supervises Home Health Aide every fourteen (14) days in order to re-evaluate the patient’s continuing personal care needs.F.Insures that skilled nursing care is being provided as established in policies and procedures and state practice act.G.Initiates preventative and rehabilitative nursing procedures as appropriate for the patients care and safety in conjunction with physician’s orders.H.Observes signs and symptoms, reactions to treatments and changes in the patient’s physical or emotional condition and reports to psychiatrist and/or medical doctor. Keeps Clinical Manager aware of these problems.I.Teaches and counsels patient and family members regarding patient care needs and other related problems of the patient at home.J.Evaluates patient care and progress at least every sixty (60) days. Documents progress in nursing care plan and notes.K.Provides continuity of care through accurate, timely reports to team members, participates in case conferences, and on-call responsibilities.L.Completes all documentation in a timely manner to insure compliance with guidelines established by state, federal, and accreditation organizations.M.Submits care plans for weekend visits in the assigned territories as soon as aware of the need for visits.N.Participates in nurse on-call system.O.Participates in patient care planning conference and staff development meetings, will provide all information needed to evaluate the need for continued coverage of each patient and the care needed to provide the most effective treatment plan and insure the highest level of quality comprehensive care.P.Strives continually to improve nursing care by broadening and deepening knowledge through attendance at workshops, conferences, professional organization meetings and individual research and reading.Q.Participates in crisis intervention as necessary.R.Participates in the planning, operation and evaluation of the nursing service.
