RN - Case Manager / Registered Nurse / Nursing We are looking for a full time RN - Case Manager to join our home health team. 2 positions - Home Health Mount Vernon, Ohio: Case Management only Delaware, Ohio: Case management and visits The Registered Nurse Manager is to coordinate the total plan of care for the home care patient. The total plan of care involves developing and maintaining long term care with service providers, patients and families of the patients. The RN Case Manager is to conduct the admission of the client, outline the skilled nursing care that will be provided, communicate with QI RN Specialist regarding the client's care after evaluating the care and progress, evaluate the effectiveness of provided services, and discharge client when  services are completed. The RN Case Manager is to emphasize that the company is providing services to maximize the health and comfort of the patient by providing home services.
