RN Case Manager TSLH Provides direct patient care in the Case Management setting. Provides assessment and planning for individualized patient care. Communicates with physicians about changes in patient's clinical condition including: patient monitoring and results of diagnostic studies. Responds quickly and accurately to changes in condition or response to treatment. Participates in performance improvement activities. This position will supports Tempe St Luke's Hospital. May have to support SLMC if needed. Provide Case Management for ER and Inpatient Units as needed. This position is full time, full benefits; exact schedule and hours to be determined.
