3- Bản sơ yếu lý lịch tự thuật nêu rõ quá trình công tác (CV)
4- Bản sơ yếu lý lịch có xác nhận của chính quyền địa phương trong vòng 06 tháng trở lại đây.
5- Các văn bằng chứng chỉ khác
6- 01 ảnh 4x6 trong vòng 06 tháng trở lại đây.

Yêu cầu khác

- Có khả năng giao tiếp tốt, sức khỏe tốt.
- Thông minh, nhanh nhẹn, trung thực, chăm chỉ
- Có khả năng làm việc tăng ca khi có yêu cầu
