BAYADA Home Health Care is seeking an energetic, self-motivated Nurse, RN or LPN to fill the position of Clinical Liaison for our Gloucester County office. As Clinical Liaison, you will be responsible for generating referrals by building relationships with case managers, social workers, discharge planners, physicians, and other community resources. You will be responsible for identification of clients eligible for home care services, coordination of intake with the office, and obtaining initial insurance authorizations for home care services as needed. Prior home care, case management or discharge planning experience preferred. Educating the health care community regarding the spectrum of home health and hospice services available through BAYADA is also an integral aspect of the Clinical Liaison role.Meet with prospective clients/families to provide education about home health serviceCoordinate start of home health services for client, referral source and BAYADA officeObtain necessary physician ordersObtain initial insurance authorizations as needed Participate in community and marketing events as neededTravel to designated facilities throughout Gloucester CountyThis position requires an RN or LPN who is able to develop and build strong, lasting relationships with referral sources, clients and families. Excellent planning, organization and presentation skills are critical.
