•** Position covers the Sandhill Cove retirement community in Palm City/Stuart, Florida. Work hours are scheduled to be 2 Saturdays per month for 5 hours per day. ***Job Description Summary:The Nurse Practitioner is responsible for providing primary care and maintaining continuity of care for residents of the community by establishing diagnosis and treatment of common acute and common chronic illness with health promotion and disease prevention.Essential Job Functions/Responsibilities:A.Provides general medical care and treatment to patients in a clinic or health center under direction of physician: Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician. B.Discusses case with physician and other health professionals to prepare comprehensive patient care plan.C.Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs. D.Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.E.Outline and maintain a process for ensuring the receipt, documentation, and conveying of results to the residents for all ordered tests and referrals. F.Perform certain laboratory studies including the drawing of blood samples, performing urinalysis, stool examination, EKG, bacteriologic and cytologic smears and cultures, skin tests, and blood glucose determinations. G.Perform therapeutic procedures including injections, immunizations, cleansing, suturing, dressing wounds and decubiti, inserting nasogastric tubes and catheters, irrigation of ear canals, and administration of oxygen. H.Diagnose and treat acute medical problems and manage chronic conditions.I.Make necessary rounds.J.Perform pre-admission physicals on potential residents for admission to the community and make recommendations for level of care. K.Issue orders for medication, treatment, and laboratory tests under the guidelines and mandates of the Board of Medical Examiners and under the direction of a physician.L.Consult with members of the care team to integrate information into treatment plan.M.Provide information and consultation to family members.N.Provide resident education and counseling.O.Maintain open communication with medical director and other physicians. P.Prescribe pharmacologic and non-pharmacologic treatment processes.Q.May require home visits within the residential community to meet patient/resident needs.
