Patient Advocate Pathfinder Home Health – The Woodlands, Texas   Our mission at Pathfinder Home Health is to help each of our patients attain their optimal health potential in order to improve their  quality of life and achieve a maximum level of independent living. Pathfinder Home Health is a growing healthcare company that supplies in home healthcare services.  We are seeking a talented, professional outside sales representative to help grow our business.  The ideal candidate will have successful documented business to business sales experience.  Some knowledge of the healthcare industry would be beneficial, but is not required. This position involves multiple face to face encounters each day with medical referral sources to generate appropriate patient referrals.  Those with the skills to manage their time and territory effectively are encouraged to apply.  This is a direct sales position and the ability to ask for and get the referral is a must.   Responsibilities will include but are not limited to the following:   Build a network of referral sources Represent Pathfinder Home Health in a professional manner Document daily call reports Communicate with colleagues in order to ensure referrals & admissions are coordinated in a timely manner Compile customer satisfaction reports and summarize findings Assist in development of marketing plans Work to increase number of referrals Act as patient advocate between referral sources and intake staff
