Job Summary:    Nursing leadership position which coordinates patient care, management and education with a multi-disciplinary health team.  Provides and manages patient care activities to patients of diverse ages as defined: birth to death. Essential Duties: Provides leadership as a role model in developing and implementing the team concept of patient care delivery. Works independently with limited direct supervision in accordance with the Iowa Nurse Practice Act, Mercy Medical Center policies and patient care standards and professional judgment. Evaluates with Quality Management/Utilization Review and Social Services appropriateness of resource utilization based on established admission, discharge and transfer criteria, initiating appropriate intervention and collaboration to bring about successful result. Demonstrates leadership skills in quality improvement activities including performance improvement processes to facilitate clinical practice changes. Provides input toward performance reviews on an on-going basis as well as annually.  May follow-up with patient at home or across the continuum. Maintains clinical competencies to serve as mentor to staff and as a clinical resource to patient care personnel. Collaborates with Unit Director for a plan for achievement and maintenance of skills/ competencies of team personnel.
