The Clinical Manager has a prime responsibility  in assuring quality and safe patient care and services to  train agency's home health patients.  The position's  responsibilities require substantial specialized knowledge, judgment and nursing skills based on principles of psychological, social, biological and physical sciences.  The incumbent is responsible and accountable for making decisions that are based on his/her educational preparation and experience in nursing supervision of field personnel. Responsibilities include the following: Co-ordinates all Intermittent department processes. Evaluates patients referred for home care to determine appropriateness for admission.  Refers patients needing other services to appropriate agencies. Complete intake form with all appropriate information.  Document new referrals onto referral log.  Obtain physician orders. Consults with family, institutions, physicians, patients, nurses, and others, while establishing a coordinated home health care plan prior to discharge.  Notifies CSR and RN Case Manager of patient acceptance/admission for home health care and provides intake data including background information, care plan, and signed medical orders. Assures all new patient information is sent to the Cincinnati office so preauthorization can be obtained for all home health agency admissions and each patient is entered into authorization system.  Provides feedback to referral sources. Assigns nursing assessment of home health patients within 24 hours of admission. Assures compliance with P.O.C.'s and physicians orders. Matches employee skills to home health patients. Secures/Coordinates equipment and supplies for use in the home when needed.  Oversee internal process:  Ensure diagnosis codes are on all OASIS as appropriate. Ensure data is entered into OCS system, reviewed by QA Coordinator, 485 is generated, reviewed by QA coordinator, and faxed to MD for signature.  Oversee medical records and the 485 tracking process.  Run recertification list every week to ensure recertifications are completed timely.  Ensure that all OASIS discharge information is entered into OCS timely to ensure proper billing.  Resolves problems as they may arise involving the client's care and service. Confers with doctor (MD) and Insurance Care Managers on patient progress and ongoing evaluation.  Provides coordination/consultation on discharge from home health services or readmission to other facilities. Provides supervision, evaluation, guidance and counseling to all home health agency staff.  Conduct routine meetings with nurses and therapists.  Performs annual evaluations for field staff employees in the skilled visit department. Oversees on-call schedule for Case Managers and PRN nurses.  Manage orientation of all new home health agency nurses. Attends staff development and in-service programs as required. Participates in ongoing chart audits of home health agency patients (skilled and therapy) to ensure compliance with orders.  General knowledge of business aspects of Interim HealthCare.
