Registered Nurse / RN Case Manager - Home Health - Full Time

Provide excellent care to homebound patients to help them remain independent and safe in the home. Teach patients to manage their disease and manage their medications too reduce visits to Emergency Room and Hospital.As a RN Case Manager on the Interim team you would be responsible for the patient assessments, development and implementation of individualized treatment plans to meet the home health care needs of each of your clients. Along with our Home Health Care Supervisor the clients needs will be determined and met. The care team could consist of a Registered Nurse, Physical Therapist, Occupational Therapist, Speech Therapist, Medical Social Worker and/or Home Health Aide. We offer flexibility in choosing your caseload. You could work part time or full time depending on the case load you want to carry.
