RN Hospice Nurse (Registered Nurse / Healthcare / Nursing) Job Responsibilities As a RN Hospice Nurse with a background in medical healthcare, you will be following up and interacting with patients/families/caregivers and employees in a courteous and professional manner at all times to ensure that a 5-Diamond experience is received by all customers that will contribute to Hospice of Michigan’s status as a world class organization. Additional responsibilities RN Registered Nurse include: Providing nursing care for @HOMe Support patients and families; providing direct, high quality, evidence-based palliative interventions to patients and families; demonstrating expertise in complex nursing care and pain and symptom management Maintaining up-to-date knowledge of methods, trends, and technologies as they relate to area of responsibility; using professional background and skills to assist the team and facilitate patient-family meeting; assisting team, attending physician, staff, patients and families in the clarification of treatment goals and the plan of care Facilitating formal and informal meetings, in collaboration with the interdisciplinary team (IDT) regarding patient and family communication and decision making; providing ongoing patient/family instruction; informing the manager or designee of unusual or potentially problematic patient/family issues Communicating with attending physicians as needed and as appropriate; informing primary physician of services provided and collaboration with another physician if ordered; providing summary report to primary physician upon request Serving as liaison between the @HOMe support team, the hospice team and the attending physician. Documents patient care following established standards of documentation practice Completing initial history and assessment and documents problem areas to be developed in the plan of care within twenty-four (24) hours; supervising, instructing and evaluating assistants Participating in/attends @HOMe Support orientation and training programs for professional, ancillary, and volunteer staff; participating in on-call coverage with the interdisciplinary team for @HOMe patients, seven days a week Performing assessments and symptom management consultations with HOM teams as applicable; participating in activities to promote personal, professional and organizational goals and growth; participating as requested in the development and review of appropriate protocols and processes that support appropriate referrals and consistent care; participating in process improvement activities Developing and maintaining a proficiency in the use of computers to accomplish the following activities: documentation and transmission of data to agency approved system Following consistently all departmental and organizational protocols and practices to ensure 5 Diamond service excellence is achieved with both internal and external customers, and that boundaries within the scope of practice are not exceeded Adhering to @HOMe Support and HOM policies and procedures and all regulatory and legal requirements; adhering to the C.A.R.E.
