NURSE NAVIGATOR, SENIOR CARE

Serves as a coordinator, coach and clinician to the senior patient population during the continuum of care within the Emergency Department setting. He/she will work closely with clinical staff utilizing an interdisciplinary approach to care. Coordinates the transition to the home environment (Private home, Independent Living, or Personal Care living environments). Establishes a Partner in Care relationship with patients. Schedules follow up physician appointments, and testing. Contacts the discharged patient within 24 hours of discharge telephonically to discuss the transition. Works in collaboration with the physicians, nursing, and the health care team to ensure that the discharged senior patient population quality of care and outcomes are maintained at the highest level. Serves as the primary point of contact for the discharged senior patient population in Emergency Department. Requires a Registered Nurse in the state of Pennsylvania, BSN in nursing required, MSN or MHA preferred, CCM perferred. Requires previous Emergency Department clinical experience as well as previous experience in Case Management in the acute hospital setting and/or the insurance health plan setting.
